Mounjaro, known scientifically as tirzepatide, is a prescription medication originally developed for managing type 2 diabetes. However, it has gained widespread attention for its significant weight loss benefits. The medication was introduced in India in March 2025 and is administered as a once-weekly injection.
Recently, comedian Aishwarya Mohanraj opened up about her dramatic weight loss journey in a detailed 15-minute YouTube video, where she discussed using Mounjaro to lose 22 kilograms.

Aishwarya Mohanraj’s Weight Gain And Health Challenges
Aishwarya shared that she was naturally thin during childhood and early adulthood. At the beginning of 2021, she weighed around 51 kg. However, in her late twenties, particularly around the time of her relationship and marriage, she began gaining weight. By the time of
her wedding, she weighed approximately 64 kg, and within six months, her weight increased further to around 72–74 kg.
She clarified that her weight gain was not purely lifestyle-related. She was diagnosed with PCOD (Polycystic Ovarian Disorder) and hypothyroidism, both of which run in her family. These hormonal conditions contributed to weight gain and made losing weight significantly more difficult.
To manage her PCOD, she was prescribed medications including birth control pills and later metformin, as her condition was linked to insulin resistance.
Attempts At Weight Loss Before Mounjaro
Before opting for medication-assisted weight loss, Aishwarya tried several methods:
- Daily workouts
- Eliminating junk food
- Increasing protein intake
- Walking 10,000 steps every day (even on rest days)
- Cosmetic procedures such as CoolSculpting, HIFU, radiofrequency treatments, Morpheus, and Dermage
Despite her consistent efforts, she struggled to see sustainable results. Her situation became more challenging when she developed plantar fasciitis, which limited her ability to stay physically active.
After exhausting multiple approaches, she decided to begin treatment with Mounjaro.
Her Experience With Mounjaro
Aishwarya explained that within the first month of starting the weekly injection, she lost 4 kg. She continued the medication for six months, during which her weight decreased from 74 kg to 52 kg.
She described the transformation as rapid and intense. However, she was transparent about the side effects she experienced, particularly in the initial stages. These included:
- Severe nausea in the first month
- Noticeable hair loss
- The high cost of treatment (approximately ₹4,000 per injection)
Given that the medication is taken weekly, the overall expense can be significant over several months.
How Does Mounjaro (Tirzepatide) Work?
Tirzepatide works by mimicking two key hormones naturally produced in the body after meals:
- GLP-1 (Glucagon-like peptide-1)
- GIP (Glucose-dependent insulinotropic polypeptide)
These hormones help regulate blood sugar levels and appetite.
Mechanism of Action
When administered, Mounjaro activates GLP-1 and GIP receptors in the body, enhancing their natural effects. This results in:
- Improved blood sugar regulation
- Increased insulin secretion
- Reduced appetite
- Decreased food cravings
- Enhanced feeling of fullness
Additionally, the medication slows down gastric emptying, meaning food remains in the stomach longer. This prolongs satiety after meals and reduces post-meal blood sugar spikes.
Because of these combined effects, many individuals experience reduced calorie intake and gradual weight loss.
Who Should Avoid Mounjaro?
Although Mounjaro can be effective, it is not suitable for everyone. It must only be taken under medical supervision. Certain individuals should avoid this medication:
1. History of Medullary Thyroid Carcinoma (MTC)
People with a personal or family history of MTC should not use Mounjaro. Animal studies have shown an increased risk of thyroid C-cell tumors.
2. Multiple Endocrine Neoplasia Syndrome Type 2 (MEN 2)
Individuals with MEN 2, a genetic condition linked to endocrine tumors, are advised against using this drug.
3. Pregnancy and Breastfeeding
Animal studies have indicated potential risks to the fetus, including skeletal abnormalities. It is not recommended during pregnancy or while breastfeeding.
4. History of Pancreatitis or Severe Gastrointestinal Disorders
Since the drug slows gastric emptying, it may worsen conditions such as:
- Gastroparesis
- Inflammatory bowel disease
- Previous episodes of acute pancreatitis
5. Type 1 Diabetes
Mounjaro is not approved for type 1 diabetes and may increase the risk of hypoglycemia if combined with insulin therapy.
6. Severe Kidney Impairment
Gastrointestinal side effects such as nausea, vomiting, and diarrhea can lead to dehydration, which may pose risks for individuals with kidney disease.
7. Diabetic Retinopathy
Rapid blood sugar improvements may temporarily worsen diabetic retinopathy, so careful monitoring is necessary.
Important Considerations
While Mounjaro has shown promising results in both diabetes management and weight loss, it is a prescription medication and should only be used after consultation with a qualified healthcare professional. Weight loss drugs are not a substitute for lifestyle changes, and long-term results depend on diet, exercise, and proper medical supervision.
